I guess what he's trying to say is that "How to" sounds like a tutorial/guide and is grammatically incorrect as a question. The correct title of this thread would be "How do I make lava?", or at the very least a grammatically incorrect "How to make lava?" but without the question mark, it definitely sounds like a guide.
Anyway, to answer the question, lava is added the same way as water. There are a bunch of water types (named by tileset) and I think 2 of those are lava (icon reflects it)

I've seen this 1 million times when people ask in games on starcraft "How to play" and then I see a few threads that say "How to such and such". I find it slightly annoying that nobody understands the more correct statement "How do I blah blah (Add a question mark)". It's especially annoying when it's in a thread title because it's the first thing you see and it looks like a tutorial and not a question. I'm just pointing this out so anybody that reads this doesn't make that same mistake again. That's all.
I'm confused. How is "How + complete verb" incorrect grammar? I'll have to agree with you that a question mark needs to be added to prevent it from looking like a tutorial, but I don't understand your beef with 'how to'.
