You would need to create a model in 3ds max. I am not sure if the current exporter can handle the particular type of model needed to make a decal, you would have better luck asking in the thread in the art forum.
Before I go through the trouble of doing that, would you mind giving me some tips on how I can experiment w/ existing Decals to see if I can get the result I want before spending the time trying to design custom decals? Thanks!

Also, if so, can you use ground textures outside the limited amount?
No and no. You can workaround by using a decal, though, which is a splat actor that covers the ground.
