As promised the system for requesting a Beta key is in place.

We'll be getting keys out to people as soon as we can, and just a reminder you must be an active WoW author to receive one of these keys. We'll be manually verifying eligibility as well.
Note: if you see a message about and error when placing your order, please just try again.

Going forward, anyone who wants to sign up for a key must have a certain minimum threshold for the popularity of their addons in order to be approved. We'll be using the same metrics as we do for the rewards program for this, although you don't have to be signed up for the rewards program to be considered.
We are leaving it open to new users, however, by default you'll have to wait until your addon reaches the threshold of popularity before you're eligible. Also, at this point we're leaving the giveaway open to team members on addons, not just the manager.

Update to the update: It turns out that at least some US players are _also_ having the issue redeeming keys. I'm going to keep handing them out though as I've been told that they should work in the near future.

I'm about to start sending out the new batch of keys. However, there is currently an issue were EU players cannot redeem them. We've been told that there is a fix coming soon. As soon as I receive word that these work for EU players I'll post another update here.
Haven't given out any more keys since last week guys. Waiting on more. If I reject your order, I'll change it's status to "Canceled".
I'll let you all know as soon as I receive more keys and start distributing them again.

From Kaelten's comment " likely to eligible for both rewards points and to receive a beta key." I'm presuming that if you get curse points for your addons, you're eligible. Obviously that's just my guess though, don't quote me on that :P
As far as position in the queue goes, I don't think it's being processed in order, possibly with authors with more popular projects getting priority - my mate who's got a fair few projects applied a few hours after me and got his key almost instantly, whereas I'm still waiting.
Sigh. Sucks I didn't even knowa bout this until yesterday. Wish there had been improved notice like an email. Always get emailed when useless keys like MoH are available :( Does the curse mailer allow targeting like to authors only like it does for premium only?
Also, i'm curious about Jims question. If you help with an addon but just weren't added to it yet. Would it affect anything if added to it now? Or number of addons doesn't really matter, just most popular out of them chosen as a guideline?
Also WoWI gives you an idea of where you kinda sit in queue/amount of keys given. Any update on that here?
@Fakeh I PMd Kaelten to see if I'd been turned down as mine's been on "placed" for a while, and he said It was just waiting on more keys to come in, so I guess you're most likely in the same boat.
Thanks for organising this. Even if my addon doesn't meet the threshold I'll be glad that many popular mods will work come patch day :) A quick question, is it normal to wait 48hours plus for approval? If not is there anything I can do on my end?

I won't be releasing the exact details of the threshold mechanisms. But for all intents and purposes. If you write a addon and it's used by a good number of people and is downloaded regularly you're about 99.99% likely to eligible for both rewards points and to receive a beta key.

I also want to let people know that the download counts on curse.com are kinda misleading relation to your popularity. It's a known issue on our side, and fixing it is in the queue, but downloads from the client aren't counted at the moment. This results in significantly lower download counts showing on the website.
Note: That bug does not affect the metrics we're using to determine popularity.
@Ackis I meant that I helped on development with an addon that I wasn't listed as a co author on, but I had a word with the main author anyway and he amended it, just in case that affects my application.
There is "Resubmit for approval" but I can not find a Report button. I found the link in the sourcecode now but when I search for "report" on the page I see no link. o.O
Awesome that you're offering beta keys again. Just wish I'd spotted this a couple of days ago! ;)
I'm curious how the threshold is calculated as well. I've had an addon in development and registered on Curseforge for almost 2 months. In that time many of my guild members have been using it and loving its functionality. I know I've also had downloads from players not in our guild and probably not on our server. I chose Curseforge as my development site in May because of it's awesome pages, integration with Curse.com (and it's amazing downloader), and the ease of use of Curseforge's SVN repository.
However, my addon's total downloads is just under 250.
And, I have no idea what Curse points are.
Any chance I could even qualify for a beta key?

This is not the forum to discuss issues with your addons approval/rejection. There is a report mechanism for that.

I could throw out the number to you, but there is no context so it wouldn't make sense. If I said the threshold was 50, would that make any sense? I also don't think it will be released because it ties into the rewards system.
err.. my addon page says: Your AddOn only works for French-speaking people. Are you sure? Never tested it for french but it works 100% with German Client and should work with English and other languages as well. o.O
But... what is the threshhold?
The threshold is quite reasonable. When I was looking at the data, my initial gut reaction was a threshold about 10x higher than it actual is.

Yeah I'm also curious what the "rewards program" threshholds are, and how they are determined?
Out of interest, what's the threshold on this, or is it not a figure available to the public? Would be handy to know whether I should ask for recognition on a more popular addon I worked on, or whether it's not worth the hassle.
